Appointments and Scheduling

We do our best to keep to appointed schedules, but medical emergencies and unexpected surprises do occur. If that happens and it delays your appointment, we promise to keep you informed and let you know of other options to fit your needs. If you need to cancel an appointment, please do so as far in advance as possible.

Please bring the following items with you to your appointments:
Insurance card(s) – authorization/referral, if necessary – x-rays/MRI related to problem – picture identification

The patient is responsible to notify the office of any changes to their address, phone number (home, work or cell) and insurance. However, every year we will ask you to complete new demographics and provide a new history.

ARRIVAL PRIOR TO APPOINTMENTS

We ask that new patients arrive 30 minutes prior to your initial appointment time to allow ample time for the staff to verify and finalize your records. If you are an existing treating patient with record changes, please arrive 10 minutes prior to your appointment to allow time for the staff to make the necessary changes to your records.

PAYMENT OPTIONS

We accept Cash, Check, MasterCard, Visa, Discover, and American Express. Like most physician offices, we request payment at the time of your service unless we are a participating provider for the insurance company. Since most insurance companies do not reimburse the total amount due, you will be responsible for any charges that are not covered under your plan.

INSURANCE

Our office will bill your insurance company for you. However, you are responsible for any unmet deductibles, co-insurance or non-covered benefits that may occur. Co-pays are payable at each appointment and at the time of check-in.
If your insurance requires a referral or authorization, it is your responsibility to obtain it from your Primary Care Physician. This referral or authorization number is required for your initial orthopaedic evaluation and treatment. As an existing treating patient, the same will apply if your visits have expired, for continued treatment as necessary. If you arrive for your appointment without an authorization, we may be required to reschedule your appointment.

MEDICAL RECORDS

If you need to request certain medical records from our office, please be aware that the request could take ten days to two weeks before you receive the information. We will make every effort to process your request for records as quickly as possible. If you are requesting records for another physician, it is faster for us to send the records directly to the physician. Patients wanting their records themselves will be charged $1.00/page.

PRESCRIPTIONS

Prescriptions may be refilled during normal business hours. Prescriptions will not be refilled after hours or on weekends.

TREATMENT OF MINORS

A parent or legal guardian must accompany new patients under the age of 18 at their initial visit. The parent or legal guardian may designate someone to accompany an existing patient during follow-up visits, if it is specified in writing and provided to the office.

PHYSICAL THERAPY

Physical therapy is available through our office. At present, we can schedule you for a Monday, Tuesday, Wednesday, Thursday (all day) or Friday (AM) appointment.
